BASIC Requirements for Student Visa Application
Important Note: Documents are only valid ( 3 ) months upon issuance.
- Duly Completed Blue Form
- Transcript of records
- Diploma
- Certificate of employment from ALL past and present job
- JLPT or NAT or JTEST Certification at least N5 level and Basic Nihongo Language Certificate (180 Hours or more)
- Proof of relationship to Sponsor (Birth certificate, etc.) *Issued by PSA
- Marriage Certificate (if applicable)
*Issued by PSA - Specimen Signature
- Picture (4×3) Taken within 3 months ― 6 pcs.
- AFFIDAVIT OF PROOF OF RELATIONSHIP with Sponsor

- Certificate of Employment or Business Permit (DTI)
*INDICATE YEARLY SALARY ( WITH BONUS/BENEFITS ETC…) - Latest ( 3 ) years’ worth of Income Tax Returns
- Bank Certificate issued from Bank
- Latest ( 3 ) years’ worth of Bank Statement/History issued from Bank
- Proof of relationship to Applicant (Birth certificate, etc.)
*Issued by PSA - Marriage Certificate (If applicable)
